Fire Pump Room: On or about January 21, 2021, access to control valves for the fire suppression system was impeded by the storage of items in front of them, subjecting employees to prolonged exposure to fire conditions in the event access to the controls was necessary.

No. HC216658, prolonging employee exposure to energized equipment in the event the switchboard would need to be accessed. In accordance with 1910.303(g), Table S-1, Condition B, the required distance is 42 inches.

29 CFR 1910.106(b)(4)(iv)(g): The inlet of the fill pipe shall be located outside of buildings at a location free from any source of ignition and not less than 5 feet away from any building opening. The inlet of the fill pipe shall be closed and liquidtight when not in use. The fill connection shall be properly identified. Fire Pump Room, Diesel Tank: On or about January 21, 2021, the fill pipe provided on the 281 gallon fire pump tank was located inside the building. The tank contained diesel fuel, a Category 3 flammable liquid.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical. Fire Pump Room: On or about January 21, 2021, a diesel tank with a volume of 281 gallons was found to be missing appropriate labeling, thereby not informing employees of the associated flammability hazards, skin irritation hazards, inhalation hazards, carcinogenic hazards and possibly exposing employees to the same.
