29 CFR  1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  (a) Shop - Employees used a shop-made fifth wheel attachment on the forks of the Caterpillar sit-down rider diesel forklift to move truck trailers.  The employer did not get the forklift manufacturer's prior written approval to use this attachment, exposing employees to the hazard of overloading the forklift, on or about January 21, 2021.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) Shop - The Hyd-Mech S-20A horizontal band saw, used to cut metal, did not have guarding on an 18.5-inch portion of the saw blade between the left wheel guard and the adjustable left guide rolls, exposing employees to getting cut by the saw blade, on or about January 21, 2021. 29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a) Shop - The employer had metal junction boxes with knockouts combined together to form a pendant attached to a flexible electrical cord that hung down from the ceiling.  The receptacles on this pendant supplied power to a pedestal grinder and a sander.  The employer did not have these junction boxes installed in accordance with the boxes' listing, labeling, or certification in that the employer did not have these junction boxes with knockouts securely and rigidly mounted to the building or other support structure, exposing employees to injurious contact with electrical current, on or about January 21, 2021.
