(b) UTCRAS, Machine shop, Building O: On or about January 21, 2021, the employer did not ensure that the exit route inside Building O was maintained free of obstructions. Items such as but not limited to cardboard boxes stored on a pallet and other metal parts were obstructing emergency exit route, exposing employees to fire hazards in case of an emergency.

(b)	UTCRAS, Stations # 133/134 and #135, Building O: On or May 5, 2021, the employer did not take out of service the powered industrial trucks that needed tires replacement and maintenance. Employees were exposed to severe injuries while using propane forklifts 80 Nissan (Model: MCUG1F2F361 V) and Hyundai (Model: 25L-7A), with damaged tires showing missing tire chunks, tearing and cracking.
