29 CFR  1926.303(b)(1):Grinding machines were not equipped with safety guards in conformance with the requirements of ANSI B7.1-1970, Safety Code for the Use, Care, and Protection of abrasive wheels, and paragraph (d) of this section.   a) On or about January 21, 2021, while on the roof, an employee used a Craftsman CMEG100 hand grinder to cut roof tiles while missing the guard.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   a) On or about January 21, 2021, while on the roof, employees worked without adequate fall protection while performing roofing operations at a height of approximately 30 feet above the ground.
