29 CFR  1926.300(c): Employees using hand and power tools and exposed to the hazard of falling, flying, abrasive, or splashing objects, or exposed to harmful dusts, fumes, mists, vapors, or gases were not provided with the particular personal protective equipment necessary to protect them from the hazard.    a) On or about January 21, 2021, at the front of the building, an employee operated a Dewalt hand grinder performing work to the exterior of a building while not wearing eye protection.

29 CFR  1926.303(b)(1):Grinding machines were not equipped with safety guards in conformance with the requirements of ANSI B7.1-1970, Safety Code for the Use, Care, and Protection of abrasive wheels, and paragraph (d) of this section.   a) On or about January 21, 2021, at the front of the building, an employee operated a Dewalt hand grinder missing the guard while performing work to the exterior of a building.

29 CFR  1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.   a) On or about January 21, 2021, at the front of the building, while working from a scaffold not fully planked, an employee was performing work to the exterior of a building approximately 10 feet above the ground.

29 CFR  1926.451(g)(1)(vii):Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   a) On or about January 21, 2021, at the front of the building, while working from mobile scaffolds two employees were not protected by complete guardrails or wearing fall protection, the employees were performing work to the exterior of a building approximately 10 feet above the ground.
