29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Saw # 8 in production area; On or about January 21, 2021, an effective means of guarding was not in place on the # 8 Oliver saw to prevent employees from accessing the blade and nip points between the clamp and material during clamping and cutting operations.

29 CFR 1910.212(a)(2): Machine guard(s) on machine(s) were so installed that they offered accident hazards in themselves: (a) Saw # 8 in production area; On or about January 21, 2021, employees were exposed to caught-by/crushed-by hazards when the hood guard/material hold down on the # 8, Oliver saw unexpectedly initiates the clamping cycle by lowering the clamp to the cutting table.
