29 CFR  1926.453(b)(2)(v):A body belt was not worn and a lanyard was not attached to the boom or basket while working from an aerial lift.  On or about January 21, 2021, two employees were performing work from an aerial lift up to approximately 20 feet high without using fall protection, exposing the employees to a fall hazard.
