29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: (a) Alliance Colorado Builders, LLC at 1160 E. 18th Ave, Denver, CO 80218: On and before January 21, 2021 the controlling employer did not ensure that subcontractor employees were protected from falling through holes by personal fall arrest systems, covers, or guardrail systems erected around such holes in that a manhole cover for the sand/oil interceptor on level B2 was missing and the wood cover over the Level 2 utility chase floor opening had gaps on either side of approximately 10-12 inches. These conditions exposed subcontractor employees to fall hazards of approximately 6-10 feet to the lower level.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers: (a) Alliance Colorado Builders, LLC at 1160 E. 18th Ave, Denver, CO 80218: On and before January 21, 2021 the controlling employer did not ensure that subcontractor employees were protected from stepping into or through holes by covers in that a sewer cover (approximately 5 inches diameter) to a floor hole on Level B2 was missing and the wood cover over the Level 3 utility chase floor opening had a gap on the front side of approximately 5 by 24 inches. This condition exposed subcontractor employees to tripping in or stepping in fall hazards due to missing or inadequate covers.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: (a) Alliance Colorado Builders, LLC at 1160 E. 18th Ave, Denver, CO 80218: On and before January 21, 2021 the controlling employer did not ensure that subcontractor employees had proper access to the outside ground level at the building's east side, in that there was a break in elevation of 27 inches from the doorway to the ground level. Employees stepped up and down onto two cardboard boxes on a daily basis. This condition exposed subcontractor employees to trip and fall hazards of 2 feet 3 inches.
