29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:  On or about January 13, 2021, employees were exposed to injuries when performing maintenance and servicing on equipment, including but not limited to, Glass Line 2 Washer, because the employer did not conduct an annual inspection of the machine specific procedures, to ensure the procedures and requirements of this standard were followed.

29 CFR 1910.147(c)(6)(ii): The employer did not certify that periodic inspections of the energy control procedures had been performed:  On or about January 13, 2021, employees were exposed to injuries when performing maintenance and servicing on equipment, including but not limited to, Glass Line 2 Washer, and the employer did not certify periodic inspection had been performed.  The certification shall include identification of the machine or equipment, the date of the inspection, the employees included in the inspection, and the person performing the inspection.
