29 CFR 1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used: On or about January 25, 2021, employees engaged in scaffold operations were exposed to fall hazards in excess of 10-ft. for climbing the scaffold walk through frame to gain access to upper levels.

29 CFR 1926.503(c)(3):Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill. On or about January 25, 2021, employees engaged in scaffold operations on the building were exposed to fall hazards in excess of 10-ft. due to inadequacies in fall protection training. Arrow building Corp. was previously cited for a violation of this occupational safety and health standard 1926.503(c)(3), which was contained in OSHA inspection number 1467260, citation number 01, item number 002 and was affirmed as a final order on May 7, 2020, with respect to a workplace located at 919 N Oregon St., El Paso, TX 79901.
