A) On or about January 14, 2021 at the 204 Cedar Ridge Drive, Glastonbury, CT job site: The employer used a scaffold pulley wheel to transfer bricks and masonry supplies, including bags of sand and cement, to the roof edge, approximately 8 feet from the ground level. The scaffold was not secured to prevent tipping when the load was added.
