29 CFR 1910.134(f)(2): Employees using tight-fitting facepiece respirators were not fit tested whenever a different respirator facepiece (size, style, model or make) was used: (a) Roosevelt Rehabilitation & Healthcare Center located at 7800 Bustleton Avenue, Philadelphia, PA: On or about January 25, 2021, the employer did not ensure that employees required to use respirators were fit tested with the same make, model, and size whenever new respirator was used at the site. The employees were required to wear N95 filtering facepiece respirators such as 3M 1860/8210, HDX H950s/H950, and Zhong Yi Mask ZYB-11, to protect against the SARS-CoV-2 virus while providing care to suspected and confirmed positive COVID-19 residents.

29 CFR 1910.134(m)(2)(i): The employer did not establish a record of the qualitative and quantitative fit tests administered to employees which included the information required by 29 CFR 1910.134(m)(2)(i)(A) through (m)(2)(i)(E): Roosevelt Rehabilitation & Healthcare Center located at 7800 Bustleton Avenue, Philadelphia, PA: (a) On or about January 25, 2021, the employer did not maintain fit test records for the qualitative fit tests for employees who were fit tested at other facilities. The employer required employees to wear N95 filtering facepiece respirators to protect against the SARS-CoV-2 virus while providing care to suspected and confirmed positive COVID-19 residents. (b) On or about January 25, 2021, the employer did not ensure that fit test records included all of the required information such as but not limited to the type of the fit test performed, the date of the fit test and the pass/fail results. The employer required employees to wear N95 filtering facepiece respirators to protect against the SARS-CoV-2 virus while providing care to suspected and confirmed positive COVID-19 residents.
