29 CFR  1904.39(a)(1):The employer did not report within 8-hours the death of an employee resulting from a work-related incident:  a)  On or about January 23, 2021, an employee felling trees in a logging operation was crushed when the tree he was felling struck or became entangle in a second tree, the top of the second tree broke off falling back toward the employee and it struck him. On January 24, 2021, the employee died as a result of his injuries. The employer did not notify the fatality to OSHA within 8 hours. OSHA became aware of the fatality through a media referral on January 26, 2021.  ABATEMENT CERTIFICATION REQUIRED
