29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment. a. 100 Industrial Drive - Unit 100 Jersey City, NJ (Warehouse Kitting Area) Employees working throughout the facility and specifically in the kitting area were exposed to hazards associated with being struck by heavy forklift traffic as forklifts were operated in close proximity to pedestrians and the employer had not certify that a workplace hazard assessment had been completed. Violation occurred on or about 11/28/2020
