29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.  a) Production Area, Black Slitter Machine: On or about January 27, 2021, and at times prior, an employee was exposed to struck-by and caught-in hazards, in that the energy control procedures for the Cut Table Slitter # 2 (Black Slitter Table) did not clearly and specifically outline the steps for shutting down and isolating hazardous mechanical energy.

29 CFR  1910.304(f)(1)(v): The employer did not ensure that fuses and circuit breakers were so located or shielded that employees will not be burned or otherwise injured by their operation.    a) Production Area, Black Slitter Machine: On or about January 27, 2021, and at times prior, an employee tasked with servicing the Cut Table Slitter # 2 (Black Slitter Table) was not protected from electrical burns, in that the employee was in close proximity to unshielded fuses and circuit breakers when disconnecting the hazardous energy source (electricity) .

29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.   a) Production Area, Slitter Machine Number Two: On or about January 27, 2021, and at times prior, an employee was exposed to trip and fall hazards, in that a 1/2 inch metal electrical conduit installed on the floor was not properly covered.
