29 CFR 1926.502(i)(2):All other covers shall be capable of supporting, without failure, at least twice the weight of employees, equipment, and materials that may be imposed on the cover at any one time: a) Lockheed Martin building under construction around hole/curb of B-3 exhaust fan (Southside); on November 20, 2021 and times prior the creating employer Smith's Inc of Dothan failed to ensure that employees of Phoenix II Contracting LLC were protected from falls of about 30 feet by not ensuring that the hole cover was capable of supporting, without failure, at least twice the weight of employees, equipment, and materials that may be imposed on the cover at any one time.
