Employees were exposed to caught-in, and struck-by hazards.

29 CFR 1910.147(c)(4)(ii)(D):The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures: On or about January 24, 2021, and at times prior thereto, the employer did not ensure the procedures included the release of all residual energy and verification of all energy sources had been eliminated prior to allowing the #1 Billet Saw operators to enter the back of the unguarded rotating saw blade area. Employees were exposed to being caught-in, crush-by and amputation hazards by the unguarded belt and pulley when residual energy still existed.
