29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) On or about January 27, 2021, in the Warehouse, employees were required to operate powered industrial vehicles, such as the Raymond Equipment Forklifts, Model #7520 DR32TT, S/N 752-17-BD62805. The employees had not been provided training including formal instruction, practical training or an evaluation of the operator's performance, thereby exposing employees to struck-by and crush hazards.
