29 CFR  1910.147(a)(3)(i):The employer did not utilize procedures for affixing appropriate lockout devices or tagout devices to energy isolating devices, and to otherwise disable machines or equipment to prevent unexpected energization, start up or release of stored energy in order to prevent injury to employees:  a. At the waste yard near the maintenance shop located at 5570 Korbin Ave., Rockledge, FL: On or about February 1, 2021, employees were exposed to caught-in-between and crushing hazards, in that; hydraulic system repair and / or maintenance of a 2005, Kenworth 200 T Model T300, Truck #117, were being performed without existing energy control procedures.

29 CFR  1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a. At the waste yard near the maintenance shop located at 5570 Korbin Ave., Rockledge, FL:  On or about February 1,  2021, employees were exposed to caught-in-between and crushing hazards, in that; hydraulic system repair and / or maintenance of a 2005, Kenworth 200 T Model T300, Truck #117, were being performed, without an existing energy control program.

29 CFR  1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  a.  At the back exit/entrance to the maintenance shop located at 5570 Korbin Ave., Rockledge, FL:  On or about February 1, 2021, employees were exposed to an electrical shock hazard, when walking and working next to a power disconnect box that had a missing cover.
