29 CFR 1926.451(g)(1)(ii):  Each employee on a single-point or two point adjustable suspension scaffold was not protected by both a personal fall arrest system and guardrail;  On or about, February 1, 2021, at the jobsite located at 1301 NE 7th St., Hallandale Beach, FL 33009.  An employee was exposed to a fall hazard of 36 feet approximately to ground while working from a two point suspended scaffold without being tied-off.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  On or about February 1, 2021, at the jobsite located 1301 NE 7th St., Hallandale Beach, FL 33009.  An employee was not wearing safety glasses while roller painting with water-based paint, exposing employee to a chemical hazard.
