A written hazard assessment certification had not been completed to certify the PPE needs and requirements for the employees operating the portable electrical undertrack conveyor.

29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: At a worksite located at 102 Grand Avenue, Forest City, Missouri 64451, employees were exposed to caught in and rotating parts hazards from the portable electrical undertrack conveyor. The portable electrical undertrack conveyor was not sufficiently guarded to prevent operators from being caught in, and contacting rotating parts of the machine.
