On or about 2/2/2021, near the entrance of the Sheriff's office, employees were not protected from falling 11 feet while installing tarps on scaffolding being used to create a wind-debris barrier.

29 CFR  1926.451(f)(3):Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  a.  On or about 2/2/2021, near the entrance of the Sheriff's office, two employees were directed to secure tarps that were acting as a wind-debris barrier along a walkway that was enclosed by scaffolding. The scaffolding had not been inspected by a competent person before the start of the workday and the scaffold did not have guardrails installed or a safe means of access, thus exposing the employee to a 11 foot fall hazard.

29 CFR  1926.454(a):The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  a.	On or about 2/2/2021, near the entrance of the Sheriff's office, employees were directed to secure tarps that were acting as a wind barrier along a walkway that was enclosed by scaffolding. The exposed employees had not been trained on the hazards of working on and from scaffolding and were not protected from falling 11 feet to ground below.

29 CFR  1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  a. On or about 2/2/2021, near the entrance of the Sheriff's office, employees used a portable 10-foot platform ladder to access the top of level of scaffolding, which measured 10 feet 7 inches high itself. The ladder was not secured in place and the employees were not provided a safe method of mounting and dismounting the ladder.
