29 CFR  1926.304(d):Guarding. All portable, power-driven circular saws shall be equipped with guards above and below the base plate or shoe. The upper guard shall cover the saw to the depth of the teeth, except for the minimum arc required to permit the base to be tilted for bevel cuts. The lower guard shall cover the saw to the depth of the teeth, except for the minimum arc required to allow proper retraction and contact with the work. When the tool is withdrawn from the work, the lower guard shall automatically and instantly return to the covering position.  (a) South side of house - On or about February 2, 2021 the employer did not ensure that the employees use the circular saw with the blade guard lowered over blade exposing employees to amputation hazards.

29 CFR  1926.451(e)(5)(iii):If the slope of a ramp or a walkway is steeper than one (1) vertical in eight (8) horizontal, the ramp or walkway shall have cleats not more than fourteen (14) inches (35 cm) apart which are securely fastened to the planks to provide footing. Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  (a) Top of house - On or about February 2, 2021 the employer did not ensure the employees framing and applying roof sheeting on the steep roof  above 6 feet were provided a means of fall protection, exposing employees to fall hazards.

29 CFR  1926.1053(b)(1):When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.  (a) North side of house - On or about February 2, 2021 the employer  did not ensure the ladder being used to gain access to the roof above 6 feet, extended 3 feet past the access point, exposing employees to fall hazards.
