29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to the joints or terminal screws: Employees were exposed to lacerations, electrical shock and fire hazards. Employees engaged in production/drilling activities in the wood rack drilling area were exposed to electrical shock and fire hazards in that employees were permitted to use the wood rack machine that lacked strain relief on both ends of the power cable: coming out of the control voltage transformer, and coming in to the 2-hand button control box to ensure effectively electrical continuity contact to the boxes. The electrical conduit cables were loose, wires pull out, exposing internal conductors and energized wires. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: Employees were exposed to lacerations, electrical shock hazards. The employer failed to conduct and document a periodic review of the energy control procedures for the shop equipment exposing employees to hazards associated with the unexpected energizing or start-up of equipment while performing servicing and maintenance activities. Equipment including but not limited to the wood rack machine, and the cold saw. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
