29 CFR  1910.147(c)(1):Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  On or about February 09, 2021, the employer did not establish an energy control (lockout/tagout) program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance, the machine would be isolated and rendered inoperable. Employees were exposed to unexpected energizing, start-up or release of stored energy while performing set-up activities on the Amada Press brake machines.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)

29 CFR  1910.212(a)(3)(ii):The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  a. On or about January 19, 2021, an employee sustained an amputation injury while operating the Amada Press Brake Machine # J010, where the employer did not provide sufficient forms of guarding to prevent the operator from gaining access to the pinch point created between dies at the point of operation.   b. On or about February 09, 2021, employee(s) operating the Amada Press Brake Machine # J010 were exposed to crushing hazards due to a insufficient training on the set up of "laser safety system" and proper operation of the machine, that allowed operator(s) to place hands/fingers in the danger zone(s) during operating cycles. The employer did not provide other of guarding to prevent the operator from gaining access to the pinch point created between dies at the point of operation.   Abatement documentation is required in accordance with 29 CFR 1903.19(d).
