29 CFR  1910.179(n)(3)(i): The load was not well secured and properly balanced in the sling or lifting device before it was lifted more than a few inches:  (a) Facility, 43 Century Drive, Ambridge, PA:  On or about February 1, 2021 the load being placed on the truck trailer was not adequately balanced in the sling while the operator attempted to seat it.  The operator picked the load up, tried to seat it, did it again and then the load came out of the sling and struck the operator knocking them to the ground.  The load fell on top of the operator's left leg causing fractures and degloving.
