29 CFR  OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment that were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that they were exposed to ejection and crushing hazards while operating rough terrain powered industrial truck without wearing a seatbelt:   On or about January 21, 2021, and at times prior, employees were exposed to ejection and crushing hazards while operating the Lull model 644B-37 rough terrain forklift which was equipped with an unused/inoperative seatbelt.

29 CFR  1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  Jobsite:  The outer sheathing of the black multi-outlet extension cord, used by employees to power an air compressor, was torn from the multi-outlet cord end exposing employees to electric shock/burns and/or electrocution.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Jobsite: a) Employees, engaged in framing work at the edge of the second-floor deck, above the garage, were not protected from falls of approximately 9ft 2-inches to grade. b. Employees, walking-working on the second floor, in the vicinity of the stair opening, were exposed to falls of approximately 9 ft posed by open, unprotected floor holes. c) Employees, walking-working on the second-floor, were exposed to falls of approximately 12-ft to grade posed by unguarded window openings at the rear of the home. d) Employees, walking/working in the garage area, were exposed to falls of over 7-ft posed by the unguarded floor opening to the basement.
