29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee was using eye or face protection when exposed to eye or face hazards from flying particles.  a) Timeline Services LLC 500 176th Ave, Broomfield, CO 80023: On and before February 4, 2021, the controlling employer did not ensure that subcontracted employees were protected from injury, in that sub contracted employees working for J Construction 17684 Eclipse St., Broomfield, CO 80023 were observed conducting framing operations on a new residential construction without the use of eye or face protection operating pneumatic nail guns. The employer did not protect employees against hazards from flying objects. This condition exposed employees to projectile hazards to the eyes.

29 CFR 1926.602(d):  Operators were not trained in the safe operation of powered industrial trucks as required by paragraph 29 CFR 1910.178(l):  a) Timeline Services LLC 500 176th Ave, Broomfield, CO 80023: On and before February 4, 2021, the employer did not ensure that forklift operators were trained in the safe operation of powered industrial trucks, in that employees  were working under suspended loads while working on the second level of a residential home. The employer did not protect employees from struck by and overhead hazards. This condition exposed employees to struck-by hazards.  Note:  The requirements applicable to construction work under paragraph (d) of this section are identical to those set forth at paragraph (l) of 29 CFR 1910.178.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   a) Timeline Services LLC 500 176th Ave, Broomfield, CO 80023: On and before February 4, 2021, the controlling employer did not ensure that subcontracted employees were protected from injury, in that sub contracted employees working for RM Construction at 17683 Eclipse St., Broomfield, CO 80023, were observed using portable extension ladders, used to access the second level of a residential home, had side rails extending 3 feet above the upper landing, and was secured. The employer did not protect employees against falls at heights over 10 feet. This condition exposes to employees to head injuries and broken bones.

This condition exposed employees to a falls over 10 feet.  c)	Timeline Services LLC 500 176th Ave, Broomfield, CO 80023: On and before February 4, 2021, the controlling employer did not ensure that subcontracted employees were protected from injury, in that sub contracted employees working for J Construction 17684 Eclipse St., Broomfield, CO 80023 were observed conducting framing operations on new residential construction without the use of personal fall arrest systems. This condition exposed employees to a falls over 10 feet.  Timeline Services LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1485251, citation number 1, item number 1 and was affirmed as a final order on  September 25, 2020, with respect to a workplace located at 1512 Bluemoon Drive, Longmont, CO 80504.
