29 CFR 1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about February 4, 2021, employees were operating compressed air powered nail guns while performing roofing operations without the use of eye protection, exposing the employees to the hazard of being struck by flying particles.

29 CFR 1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about February 4, 2021, employees were using a portable ladder to access the roof of the residential home under construction without the side rails extending at least 3 feet above the upper landing surface, exposing employees to a fall hazard.
