29 CFR 1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. a) On or about February 04, 2021 on the south/southeast side of the column, the employees were accessing the scaffold by the structure frame of the scaffold and had no ladder available on site.

29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations and/or related work from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity). a) On or about February 04, 2021, on the south/southeast side of the column, employees were working at an elevated height of approximately eighteen (18) feet on a scaffold platform, with no side rail/guardrail protected edges for fall protection.
