29 CFR  1910.178(m)(5)(ii):A powered industrial truck is unattended when the operator is 25 ft. or more away from the vehicle which remains in his view, or whenever the operator leaves the vehicle and it is not in his view.  On 2/5/2021, the Toyota Forklift 1 (Model # 8FBE2OU) in the trash pick-point area was observed running with no operator with-in 25 ft. of the running forklift.

29 CFR  1910.178(q)(7):Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected.  a) On and before February 13, 2021 an employee completed a PIV checklist stating that the PIV was being operated with out a key.  Mule #52 was running without the need for a key in the ignition.   The United States Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.178(q)(7), which was contained in OSHA inspection number 1401856, citation number 1, item number 1 and was affirmed as a final order on 11/5/2019 with respect to a workplace located at7451 TEMPLEHOF DRIVE INDIANAPOLIS, IN 46241.

29 CFR  1910.303(g)(1): Space about electric equipment. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.   a) On or before 2/5/2021 the employer did not ensure that sufficient access and working space was provided and maintained at electrical panels located at columns such as but not limited to; 8b that had poles, pallets, boxes and a metal cage in front of the panel and around the electrical panel.   b) On and before 2/5/2021, the employer did not ensure that sufficient access and working space was provided and maintained at electrical panels located at columns such as but not limited to; C4, The employer had boxes, gaylords and other debris stacked around the electrical panels resulting in limited access to the panels.  c) On or before 2/5/2021, the employer did not ensure at sufficient access and working space was provided and maintained at electrical panels located at columns such as but not limited to; Colum 15,  The employer had a PIV parked in front of the electrical panels resulting in limited access to the panels.    The United States Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(g)(1), which was contained in OSHA inspection number 1306365, citation number 2, item number 3b and was affirmed as a final order on (4/18/2019), with respect to a workplace located at 8225 Cross Park Drive AUSTIN, TX 78710.
