29 CFR  1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.   Production Area.  The Baird Four Slide Machine (Model AB412023, SN 3200-302) was not adequately guarded to prevent employees from contacting moving parts.  Abatement documentation is required for this item in accordance with 29 CFR 1904.19(d).

29 CFR  1910.147(c)(1):  The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  Entire Facility.  On or about February 5, 2021 through February 10, 2021, the employer did not have an established lockout/tagout program for the control of hazardous energy during servicing and maintenance of equipment and machinery, such as four slide machines, that included the following components:  1.  Established energy control procedures 2.  Employee training for "Authorized", "Affected" and "Other" employees (with certification /documentation of training) 3.  Periodic inspections (at least annual) of all procedures, including a review with each authorized employee as to their responsibilities under the procedures (with certification/documentation of the periodic inspections)  Abatement documentation is required for this item in accordance with 29 CFR 1904.19(d).  Abatement note:  Servicing and/maintenance is defined in 1910.147(b) as workplace activities such as constructing, installing, setting up, adjusting, inspecting, modifying, and maintaining and/or servicing machines or equipment.  These activities include lubrication, cleaning or unjamming of machines or equipment and making adjustments or tool changes.

29 CFR  1910.178(l)(4)(iii):  An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:  Entire Facility.  Employees who operated powered industrial trucks did not receive a performance evaluation (driving test) every three years, which could contribute to potential injury incidents.    There is no abatement documentation or certification required for this item.

On or about December 7, 2020 through February 10, 2021 the point of operation was not adequately guarded, exposing employees to moving parts.   There is no abatement documentation or certification required for this item.
