29 CFR  1926.451(a)(1): Each scaffold and/or scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it.    At the construction site located at 1511 NE Loop 820, Fort Worth Texas, employees utilized a Genie scissor lift with a rated capacity of 800lbs to lift material weighing approximately 1,500lbs placed across the top rails causing the load to be unstable.  The employees were exposed to falling  load hazards and Genie lift failure/collapse hazards.

29 CFR  1926.602(c)(1)(vi): 29 CFR 1926.602(c)(1)(vi):Section 603E Industrial truck(s)  American National Standards Institute, B56.1-2009, Safety Standards for Powered Industrial Trucks, as adopted by 29 CFR 1926.602(c)(1)(vi): When a truck was left unattended, load engaging means were not fully lowered:   At the construction site located at 1511 NE Loop 820, Fort Worth Texas, the employer allowed forklifts with the forks elevated to be unattended by operators.  The company was installing racking material inside the commercial building when the forklifts forks elevated exposing employees to struck-by hazards:  a)  Toyota forklift on the south side left unattended with forks raised, and  b) Toyota forklift on the southeast side left unattended with forks raised.
