29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees: San Manuel Hotel: 16th Floor: Employees conducted pipe installation and removed covers to access pipe fittings and did not replace covers after completion of tasks. Employer did not ensure covers were replaced during pipe installation operations. The employees were exposed to falls.
