29 CFR 1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a) Separator Room:  On or about February 3, 2021, employees performed response operations to isolate a valve between separators two (2) and three (3), working on a walking-working platform with unprotected sides and edges at a height of nine (9) feet and six (6) inches, exposing employees to fall hazards.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Welding Area:  On or about February 3, 2021, employees operated a Dayton 20 inch drill press for parts repair unguarded at the point of operation, exposing employees to caught-in and amputation hazards.  b) Maintenance Shop:  On or about February 3, 2021, employees operated a Delta drill press, model 70-200 for parts repair unguarded at the point of operation, exposing employees to caught-in and amputation hazards.  c) Mechanic Shop:  On or about February 3, 2021, employees operated a Dayton mill drill, model 2AC40B for parts repair unguarded at the point of operation, exposing employees to caught-in and amputation hazards.

29 CFR  1910.303(g)(1)(ii):The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:  a) I & E Room:  On or about February 3, 2021, employees performed maintenance and repair of low-voltage equipment, but the main control panel area was stored with computers, PLC's, actuators, cardboard boxes, and wood, exposing employees to electrical and fire hazards.
