29 CFR  1926.416(e)(1):Worn or frayed electric cords or cables were used:  a) San Manual Casino, 16th floor: On or before February 4th, 2021 the employer provided flexible power extension cord(s) for employees to use that were damaged exposing the inner copper wires.  Employees used the extension cord to power a 120 volt S26 Pullman Ermator HEPA Dust Extractor. Employees were exposed to electrical shock hazards.
