29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical: a) Injection Room: On or about February 4, 2021, an employee was exposed to eye and dermal hazards while handling SURFTREAT 15 contained in a 55 gallon drum located at the injection room which was not labeled as required.

29 CFR 1910.95(d)(1)(ii): Unless the employer can show that area sampling produces equivalent exposure monitoring, the employer did not use representative personal sampling to comply with the monitoring requirements of 29 CFR 1910.95(d): a) Platform C: On or about February 3, 2021 an employee was exposed to high noise levels at locations identified in a noise survey performed by the previous platform owner, XTO Energy Incorporated. A noise hazard assessment by use of representative personal sampling methods was not determined for noise exposure levels as comparison to OSHA's TWA Action Level of 85 dBA for 8 hours and did not estimate for a 12-hour extended work shift.
