29 CFR  1910.219(f)(3):Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed:  On or about February 8, 2021 the employer failed to ensure that effective machine guarding was in place on chain driven sprocket area of the dual conveyors of the McKay 96" Stretcher Level Line . Guarding was not in place to prevent employees from reaching into the pinch point area where the chain meets the driven sprocket.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   a)	On or about February 8, 2021 the employer failed to ensure that effective guarding was in place to protect employees from the in-going nip point of the dual conveyors. The hazard areas where the belt meets the pulley were not guarded on the conveyors at the exit/ stacking area of the McKay 96" Stretcher Level Line. Employees work directly in the area where the hazard is present.   b)	On or about February 8, 2021 the employer failed to ensure that effective machine guarding was in place to protect employees from the pinch point action of the overhead vacuum lift. Employees move between the exit portion of the McKay 96" Stretcher Level Line and the stacking/ banding table. There is nothing to prevent employees from entering the area while the equipment is moving between the two. The vacuum lift is operated by an employee in a different area of the machine. Employees rely on communication with that person to prevent movement of the lift while an employee is in the danger zone.   c)	On or about February 8, 2021 the employer failed to ensure that effective machine guarding was in place on the pneumatic pushers. Guarding was not in place to prevent employees from reaching into the pinch point area when the pneumatic ram is actuated and protrudes out making contact with production pieces.
