29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested whenever a different respirator facepiece (size, style, model or make) was used: a) Ascension St. Vincent's Riverside Hospital located at 1 Shircliff Way, Jacksonville, FL 32204 On or about January 16, 2021, the employer did not provide a fit test to all employees required to wear respirators. The employer required employees to wear N95 filtering facepiece respirators to protect against the SARS-CoV-2 virus while providing care to suspected or confirmed positive COVID-19 patients.

29 CFR 1910.134(h)(1): Respirators were not cleaned and disinfected using the procedures in Appendix B-2 of 29 CFR 1910.134 or equivalent procedures recommended by the respirator manufacturer: a) Ascension St. Vincent's Riverside Hospital located at 1 Shircliff Way, Jacksonville, FL 32204 On or about January 16, 2021, the employer did not ensure employees effectively cleaned and disinfected the elastomeric respirators following the procedures in Appendix B-2 of 29 CFR 1910.134 or the procedures recommended by the respirator manufacturer to protect against the SARS-CoV-2 virus while providing care to suspected or confirmed positive COVID-19 patients.
