On or about 2/1/2021, at the Lower Poplar Wastewater Treatment Facility, 1101 Lower Poplar St. Macon, GA 31201: Employees using an extension ladder were exposed to struck by and amputation hazards; in that the employer failed to identify the defects by performing an inspection before use.

29 CFR 1910.23(c)(4): Portable ladders were not used on stable and level surfaces and they were not secured or stabilized to prevent accidental displacement: On or about 2/1/2021, at the Lower Poplar Wastewater Treatment Facility, 1101 Lower Poplar St. Macon, GA 31201: Employees performing rigging operations on the Center Feed Well were exposed to fall hazards, where one employee fell in excess of 15-feet, in that the employer failed to secure or stabilize 24-foot extension ladders to prevent accidental displacement.
