29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Hershey's Coca Line Employees working with the Krones LAM-001 glue-labeling machine on the Hershey's Cocoa Line did not utilize an energy control procedure to protect employees from nip-point hazards while performing cleaning of the accumulation of cocoa powder on the machine's rotating carousel, while the machine was energized.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. Hershey's Coca Line Employees working with the Krones LAM-001 glue-labeling machine on the Hershey's Cocoa Line were not trained in LOTO procedures for the machine, which exposed the employees to caught-in and crush by hazards while cleaning an accumulation of cocoa powder on the machine's rotating carousel.
