29 CFR 1910.212(a)(5):Exposure of blades. When the periphery of the blades of a fan is less than seven (7) feet above the floor or working level, the blades were not guarded. The guard shall have openings no larger than one-half (1/2) inch. (a) Automotive Shop - On or about February 10, 2021 and at times prior thereto, employees were exposed to the rotating blades of the heater/fan that was permanently installed adjacent to the workbench.

29 CFR 1910.213(b)(3): On applications where injury to the operator might result if motors were to restart after power failures, provisions were not made to prevent machines from automatically restarting upon restoration of power. (a) Workcenter - On or about February 10, 2021 and at times prior thereto, employees were exposed to the rotating blade on the DeWalt Radial Arm Saw in that there was no anti-restart device in the event of a power failure. The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.213(b)(3) which was contained in OSHA inspection number 1261318, citation number 2, item number 2 and was affirmed as a final order on 3/20/2018 , with respect to a workplace located at the Hood River Ranger District's Parkdale Work Center.
