29 CFR 1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a. On or about February 10, 2021, at the bumper car ride, the AC power supply for the two car rides had the electrical boxes installed in such a way that they were exposed to getting wet during inclement weather conditions. The 400 amp power supply boxes were not water proof and were installed by having a wooden box which was open on one side placed over them. b. On or about February 10, 2021, at the bumper car ride operator station, an outdoor outlet box with two receptacles had a scanner and an extension strip plugged into it without having the cover sealing the box as designed. The box cover was cracked open allowing water to enter during inclement weather conditions.

29 CFR 1910.303(g)(1)(iii):The employer did not ensure that at least one entrance of sufficient area was provided to give access to the working space about electric equipment 600 Volts, nominal, or less. a. On or about February 10, 2021, at the bumper car ride, maintenance employees were expose to the hazard of having to climb over metal railings to access the power supply boxes, in that, an access entrance was not provided to the location where the two power supply boxes were installed which was enclosed with metal railings.

On or about February 10, 2021, at the bumper car ride, employees were exposed to electrical hazards, in that, temporary wiring such as a one foot flexible extension cord was permanently installed and was used to provide power to the PA system.
