29 CFR  1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports.  On or about February 3, 2021, employees performing remodeling activities were working from a scaffold platform approximately 6 feet high that was not fully planked, exposing the employees to a fall hazard.

29 CFR  1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration.  On or about February 3, 2021, and at times prior thereto, employees performing remodeling activities erected supported scaffolds without having proper training and experience and without the supervision and direction of a competent qualified person, exposing the employees to fall hazards.

29 CFR  1926.454(b):The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question.  On or about February 3, 2021, and at times prior thereto, employees performing remodeling activities erected, moved and inspected supported scaffolds without receiving training from a competent person on the applicable procedures to be followed to minimized scaffolding hazards, exposing employees to fall hazards.

29 CFR  1926.451(f)(15):Ladders were used on scaffolds to increase the working level height of employees without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph.  On or about February 3, 2021, employees performing remodeling activities were using a stepladder on a scaffold to increase their working level height to approximately 12 feet, exposing the employees to a fall hazard.

29 CFR  1926.1053(b)(4):Ladders were used for purposes other than the purposes for which they were designed.  On or about February 3, 2021, two employees used fiberglass step ladders on a scaffold platform that were closed and reclined to the wall of the residential building, exposing employees to a fall hazard.
