Flexible electrical extension cords were being used as a substitute for the fixed wiring of the structure inside of the machinery refrigeration room. a. Orange extension cord plugged into an electrical outlet near the entrance that was powering overhead lighting. b. Orange extension cord plugged into an electrical outlet near compressor log HS-12 that was powering overhead lighting. c. Orange extension cord plugged into an electrical outlet near the side entrance, which was routed through the wall and was powering overhead lighting.
