29 CFR 1926.652(a)(2):Protective systems did not have the capacity to resist without failure all loads that were intended or could reasonably be expected to be applied or transmitted to the system: (a) On February 11, 2021 and prior to; employees working in an excavation of Type B soil without cave-in protection were exposed to engulfment hazards. The excavation measured approximately 3 to 4 feet high to the first single bench. The bench measured approximately 1 to 2 feet wide and the wall to the surface was approximately 1 to 2 feet high. The width was measured at 3 to four feet. Employees were extending sewer lateral line.
