29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) Twin Falls, ID: On or about February 11, 2021 and times prior thereto, employees were attempting to remove the water loop end-cap from a pressurized C900 12 inch PVC water pipe with an impact gun and were exposed to struck-by hazards from exploding and/or shattering PVC pipe.  Workers had not been trained on the importance and methods to safely reduce pressure.
