29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: Employees performing grinding operation at the bench grinder were exposed to in-going nip points. The work rest on the bench grinder provided by the employer was 5/16 inch from the abrasive wheel. The bench grinder was discovered in the machine shop.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: Employees working in the maintenance shop were exposed to injury from electrical fire hazard. An extension cord, daisy chained with three 110 VAC power strips were plugged together to provide electrical power to a circular saw, bench grinder, and battery chargers.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): Two Flexible cords in the maintenance shop had exposed wires, exposing employees to electrical shock that could cause burns. One flexible cord was connected near the table saw and one was connected to the Dyna- Glo Delux Dual Health Technology heater.
