29 CFR 1926.501(b)(11):"Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems. a. On or about February 12, 2021, the employer did not ensure that an employee engaged in commercial construction activities six feet or more above lower level were protected from falls. An employee was working on HVAC penetrations on a steep roof at heights of over 14 feet without adequate fall protection. The following condition exposed the employee to falls of over 14 feet.

29 CFR 1926.502(d)(16):Personal fall arrest systems, when stopping a fall, shall: 29 CFR 1926.502(d)(16)(iii): be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level; a. On or about February 12, 2021, the employer did not ensure that an employee was adequately protected from falls while engaged in commercial construction activities. An employee working on HVAC penetrations on a steep roof at heights of over 14 feet with lifelines that were not adjusted to prevent a free fall to less than 6 feet, thereby exposing the employee to fall hazards of over 14 feet.
