Bay Valley Foods, LLC; northside of warehouse - On or about 2/12/21, and at times prior thereto, an exit door could not be fully opened. The door could only open 1-2 inches due to ice accumulation on the outside of the door. NO ABATEMENT CERTIFICATION OR DOCUMENATION REQUIRED

29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times: a) Bay Valley Foods, LLC , north side of warehouse- On or about 2/12/21, the employer did not ensure that the outside snow removal company that the employer had previously contracted with to remove ice and snow accumulations from the northside of the warehouse removed an accumulation of ice on the outside of an exit door which limited the opening of exit door to 1-2 inches. NO ABATEMENT CERTIFICATION OR DOCUMENATION REQUIRED

29 CFR 1910.157(c)(1):Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: a.) Bay Valley Foods, LLC; dock door wall of warehouse - On or bout 2/12/21, and at times prior thereto, a fire extinguisher was located behind storage racks and not visible from the general area. The extinguisher was located approximately 4 feet behind the storage racks. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
